At the time of your appointment, you may assist us by providing the following information:
- A treatment request from your dentist or referring doctor, if available
- A list of current medications, dosage and reason for use
- All known allergies
- All dental and medical insurance information
Your appointment may consist of a consultation and examination or treatment. The surgeon will explain your diagnosis and review your treatment options. Please feel free to ask questions at this time.
All minor patients (under the age of 18) should be accompanied by a parent or guardian. If this may be a problem, please contact our office for possible arrangements.
If your dentist has taken current x-rays, you may request they be forwarded to our office. You may also bring them with you to the appointment. If additional films are necessary, they will be taken at our office.
