After determining a plan of treatment, we will do our best to provide you with an estimated cost prior to your surgery. We ask the patient’s estimated payment be made at the time of service, unless prior arrangements have been made with the staff.
For your convenience, we accept cash, check, Visa, Mastercard, Discover, American Express and Care Credit.
It is the patient’s responsibility to provide our office with complete and accurate insurance information. Covered procedures vary with each insurance company, and our staff will assist you in determining eligibility and estimated benefits. Payment for deductibles, co-pays and non-covered benefits are due at the time of service. Please note all patients are fully responsible for all charges regardless of insurance coverage. For your convenience, we will gladly submit an insurance claim for rendered services. We will ask you to sign a financial policy reflecting acknowledgement and understanding of our policies at your initial visit.
